Identification Requirements
- Identification proof must be on the list of approved identification types.
- ID’s must be unexpired and must be issued by the state or federal government.
- The name and DOB presented on the ID must match the exact name and DOB entered on the application.
- Proof of Identification must include the consumers information shown below.
- Name
- Date of Birth
- Address(if address is not present, the consumer must provide additional proof of address.)
Eligibility Requirements
- Consumers may qualify for lifeline if they participate in one of the following programs.
- Supplemental Nutrition Assistance Program (SNAP), formerly known as Food Stamps
- Medicaid
- Supplemental Security Income (SSI)
- Federal Public Housing Assistance (FPHA)
- Veterans Pension and Survivors Benefit
- Tribal Programs (and live on federally-recognized Tribal lands)
- Consumers in California can apply with the qualifying programs below:
- Women, Infants and Children Program (WIC)
- National School Lunch Program (NSL)
- Temporary Assistance for Needy Families (TANF)
- California Work Opportunity and Responsibility to Kids (CalWORKs)
- Stanislaus County Work Opportunity and Responsibility to Kids (StanWORKs)
- Welfare-to-Work (WTW)
- Greater Avenues for Independence (GAIN)
- Low Income Home Energy Assistance Program (LIHEAP)
- Tribal TANF
- Bureau of Indian Affairs General Assistance
- Head Start Income Eligible (Tribal Only)
- Food Distribution Program on Indian Reservations
- Federal Veterans and Survivors Pension Benefit Program
- Consumers must show a card, letter, or official document, as proof that they participate in one of these programs when applying for lifeline.
- The name on the eligibility proof must match the name on the identification.
- If the last name does not match, the consumer must provide valid proof of the name.
