Identification Requirements

1. Identification proof must be on the list of approved identification types.
   1. ID’s must be unexpired and must be issued by the state or federal government.
   2. The name and DOB presented on the ID must match the exact name and DOB entered on the application.
2. Proof of Identification must include the consumers information shown below.
